Certificates (if not issued electronically) The informant must submit the following in order to register the death*: • a death registration form signed by the informant • an original medical certificate citing the cause of death, signed and stamped by a doctor or, if an inquest is held, an original coroner’s certificate and a medical death certificate stating the date and place of death If the death is registered after the statutory period of 1 year has expired, the following must be submitted: sworn statement completed death registration form confirmation of death issued by a doctor or confirmation of burial issued by a priest The statutory surcharge of EUR 30 is payable in the above case. *The informant must be one of the persons referred to in Article 48(2) of the Civil Registry Law of 2002 (Law 141(I)/2002) 4.
