Certificates (if not issued electronically) The following must be submitted in order to register a birth: • Marriage certificate or Sworn statement recognising a child born out of wedlock (for children born out of wedlock) • Copy of passport or identity card and residence permit for foreign parent(s) If the birth is registered after the statutory period of 3 months from birth has expired, the following must also be submitted: Sworn statement using the relevant form Completed birth registration form *The statutory surcharge of EUR 60 is payable in the above case. 4.
